Preferred Specimen
24 Hour Urine Container without preservative


Patient Preparation
Discrad first morning urine on day of collection and note the time. Collect all urine voided during the next 24 hours. Refrigerate the urine during entire collection. See patinet collection instructions link below. 

Minimum Volume
15 mL aliquot (20 mL aliquot)


Other Acceptable Specimens
Sterile Container (for random urine)


Instructions
  • Mix the specimen well and measure the 24-Hour volume
  • RECORD THE TOTAL VOLUME
  • Pour off 20 mL of urine into a separate plastic screw-cap container
  • Refrigerate if not tested immediately

Reference Range
No monoclonal paraproteins observed


Clinical Significance
Ideally, this test is performed after the protein electrophoresis has identified the presence of a monoclonal paraprotein.  If the patient has a known or previous M-spike, both the immunotyping and serum electrophoresis are ordered at the same time.



Urine IFE is used for the detection of monoclonal proteins as seen in monclonal gammopathies (MGUS, MM, smoldering myeloma) and amyloidosis.
