Physician Attestation of Informed Consent

Autopsy perimission A7-A1 form is required to be completed to request an autopsy. 



Permission for autopsy is to be requested by next of kin in the following order:




  1. Spouse

  2. If no spouse, an adult son or daughter

  3. If no adult son or daughter, a surviving parent of the deceased

  4. If no surviving parent, an adult brother or sister

  5. If no adult brother or sister, a guardian of the decedent at the time of his/her death

  6. Other relatives in next of kin order

  7. If it appears there are no members of categories 1-6 above, any person assuming responsibility for burial including "common-law spouse", may provide consent, after waiting 36 hours after the death

Completed autopsy report: within 30 working days
Provisional anatomic diagnosis: within 48 hours
Complicated cases: within 60 working days (these cases include extra-departmental consultation, literature review, cultures, legal consultation)
