Physician Attestation of Informed Consent
Autopsy perimission A7-A1 form is required to be completed to request an autopsy.
Permission for autopsy is to be requested by next of kin in the following order:
- Spouse
- If no spouse, an adult son or daughter
- If no adult son or daughter, a surviving parent of the deceased
- If no surviving parent, an adult brother or sister
- If no adult brother or sister, a guardian of the decedent at the time of his/her death
- Other relatives in next of kin order
- If it appears there are no members of categories 1-6 above, any person assuming responsibility for burial including "common-law spouse", may provide consent, after waiting 36 hours after the death
