Physician Attestation of Informed Consent
This germline genetic test requires physician attestation that patient consent has been received if ordering medical facility is located in AK, DE, FL, GA, IA, MA, MN, NV, NJ, NY, OR, SD or VT or test is performed in MA.
Preferred Specimen
3 mL bone marrow collected in a sodium heparin (green-top) tube
Minimum Volume
Bone marrow: 1 mL • Whole blood: 5 mL
Other Acceptable Specimens
10 mL whole blood sodium heparin (green-top), sodium heparin (royal blue-top), sodium heparin lead-free (tan-top) tube • Bone marrow collected in sodium heparin (royal blue-top), sodium heparin lead-free (tan-top) tube
Instructions
Bone marrow 1-3 mL or whole blood 5-10 mL, must contain 10% less mature myelocytes, pros or blasts. Green vacutainer (sodium heparin only). Infants 2-3 mL, (in pediatric 3mL vacutainer), must contain 10% myelocytes, pros or blasts. Ship at room temperture. Bone marrow transport medium is available upon request.
Other vacutainer tubes containing sodium heparin are acceptable. See Genetics Specimen Collection Section for detailed specimen instructions.
Specimen viability decreases during transit. Send specimen to testing laboratory for viability determination. DO NOT REJECT. If sample exceeds stability, please call laboratory at ext. 4307 for instructions.
