Physician Attestation of Informed Consent
This germline genetic test requires physician attestation that patient consent has been received if ordering medical facility is located in AK, DE, FL, GA, IA, MA, MN, NV, NJ, NY, OR, SD or VT or test is performed in MA.


Preferred Specimen
6 mL 24-hour urine collected in a plastic 10 mL tube


Minimum Volume
2.5 mL


Instructions
24-hour volume is required.
Specimen must be received at least 1 day prior to assay day for processing.
All specimens will be evaluated at Mayo Clinic laboratories for test suitability.

1. Collect a 24-hour urine specimen.
2. No preservative.
3. Refrigerate specimen during collection.
