Coccidioides Serology Panel

Test Code
10567

CPT Codes
86635, 86317 <br><strong>This test is not available for New York, Rhode Island, or Maryland patient testing</strong>

Includes
When applicable, Complement Fixation will be performed at an additional charge (CPT code(s): 86635).

Preferred Specimen
2 mL serum

Minimum Volume
0.5 mL

Other Acceptable Specimens
CSF, serous body fluids (pleural, peritoneal, pericardial), synovial fluid collected in a plastic sterile screw-cap container

Instructions
Allow serum samples to clot at room temperature. Please separate serum from cells prior to sending. Even when using serum separator tubes, serum should be transferred into another tube as the chemical composition of the serum can change and potentially affect results. For CSF and other body fluids, a sterile container without anticoagulants or other additives is appropriate.

If minimum volume (0.5 mL) sent, then the qualitative immunodiffusion screen will be performed on an unconcentrated specimen which may decrease sensitivity. Quantitative immunodiffusion will be performed to obtain a serology titer.

Interferences: Gross hemolysis or icterus may affect visualization of reactions by immunodiffusion. Gross lipemia may cause interferences with complement fixation.

Transport Temperature
Refrigerated (cold packs)

Specimen Stability
Room temperature: 72 hours
Refrigerated: 7 days
Frozen: 60 days

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Plasma • Bronchial alveolar lavage fluid • Urine • Grossly purulent fluids • Wound drainage • Swabs • Sputum

Methodology
Immunodiffusion (ID)

Setup Schedule
Set up: Mon-Fri; Report available: 7-10 days

Clinical Significance
Recommended for follow up with known positive. Qualitative (IgM and IgG) and quantitative (IgG) detection of antibodies specific to Coccidioides by immunodiffusion and complement fixation methods. Complement fixation is performed first as the quantitative test. In the event of insufficient volume or equivocal or anticomplementary results by complement fixation methods, a quantitative immunodiffusion will be performed to determine the titer. If the CF test is positive, residual specimen is frozen by us for later simultaneous testing with a new specimen to control for variability in the CF test (PRVEII). Residual specimen is saved for approximately 4-6 months. Qualitative immunodiffusion testing is performed on concentrated sample to increase sensitivity.
