Instructions
1 x 4mL EDTA (lavender-top) whole blood
9 x 4mL EDTA (lavender-top) (or equivalent) spun, create 9 x plasma aliquots (Pipette into clear 12x75 or 13x75 poly propylene transport tube w/ clear push cap)

Accession Message:
Collect: 1 x 4 mL EDTA L (lavender-top) - Keep as whole blood 9 x 4 mL EDTA (lavender-top) tube (or equivalent) - Centrifuge, pipette plasma into 9 clear 12x75 or 13x75 polypropylene transport tubes w/ clear push cap Ship all 10 tubes, 1 x Whole Blood & 9 x plasma aliquots, frozen (-10°C to -70°C)

Note 1: Patient Service Reps will need 10 collection labels or equivalent for collection tubes (10 x 4 mL or equivalent EDTA (lavender-top) drawn)
10 destination labels: 1 x 4mL EDTA (lavender-top) whole blood and 9 x EDTA plasma aliquots that will be sent to SJC.
Note 2: Will need a PSC only rule that states Draw 10 x 4 mL (or equivalent) EDTA (lavender-top) tubes; Centrifuge 9 x 4mL (or equivalent), separate off the plasma, pipette into 9 clear 12x75 or 13x75 poly propylene transport tube w/ clear push cap
Immediately freeze all upright: 1 x 4mL EDTA whole blood & 9 x EDTA plasma aliquots.
