Clinical Significance
HSV IgM is detectable in serum from >90% of patients with primary HSV infection. However, HSV IgM cannot be reliably used to diagnose acute/recent infection as it is also found in 30% of patients with reactivated HSV. This test may not distinguish between HSV-1 IgM and HSV-2 IgM due to cross-reactivity. To diagnose acute genital or mucosal HSV infection, direct detection from a lesion by culture or molecular methods is preferred. HSV IgM positivity should be confirmed by HSV-1/2 type-specific IgG testing.
