Instructions
Random urine.


Transport Container
Urine volume should be greater than 33 mL and mixed with preservative. Follow instructions included with the Urine Collection Kit OR if the kit is not used, submit a random urine specimen with a 2:1 volume of urine to 70% ethanol, PreservCyt, or CytoLyt. Keep refrigerated. Contact Client Services for the collection kit.
An unpreserved urine specimen in a screw capped container is also acceptable if received in the laboratory within 12 hours of collection. Keep refrigerated.


Transport Temperature
Refrigerated.


Specimen Stability
Less than 72 hours (specimen should be sent to laboratory within 24 hours of collection); if the urine is not sent to the laboratory immediately, refrigerate specimen.


Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Less than 33 mL; specimen greater than 72 hours old when received in the laboratory.

Clinical Significance
Urine cytology is still considered the gold standard when it comes to detection of high grade urothelial carcinoma. While associated with a low sensitivity overall for urothelial abnormality, mostly due to a lack of definitive criteria for identifying low grade lesions, it exhibits excellent specificity with few false-positive cases. Not only does it provide the ability to detect upper tract occurrences, it has a long established use in monitoring those with bladder cancer who’s disease has become more aggressive. Urine cytology still remains a cost effective test for routine screening and detection of urothelial atypia and high grade urothelial carcinoma, and combined with the addition of FISH testing for specific genetic markers urine cytology remains an effective tool in the arsenal of bladder cancer detection and monitoring.
