Limitations
If a patient is found to be Positive (Reactive) for Hepatitis B Surface Antigen, additional reflex testing will be performed based on practice guidelines recommended by the American Association for the Study of Liver Disease (AASLD).
Reflex testing includes: Hepatitis B Quantitative DNA by Real-Time PCR; Hepatitis Be Antigen; Hepatitis Be Antibody.
Specimens resulted as Positive (Reactive) are reported to the State Department of Health.

Clinical Significance
HBsAg is a protein produced by the Hepatitis B Virus (HBV) and is the earliest indicator of acute Hepatitis B. In some people, chronic infection with HBV may occur and HBsAg remains positive for a long period of time. When this occurs, other tests may be helpful to distinguish Hepatitis B carrier status, such as Hepatitis Be Antigen, Hepatitis Be Antibody and Hepatitis B DNA Quantitative PCR. Epic Test code # 4607 (Sunquest code HBsAg, No Reflex) does not reflex to any testing.
Hepatitis B surface Antigen test code (Epic # 0186) is helpful for screening in pregnancy and provides reflex testing when reported as Positive (Reactive).
For any patients considered to be at high risk for Hepatitis B infection, Physicians should order the test named: HBsAg Risk Cascade (Sunquest code HBSR / Epic # 3715). High risk includes patients with a history of IV drug abuse, history of STD’s, multiple sex partners, sex partners with known Hepatitis B disease, patients infected with HIV, homosexual contact, exposure to known contaminated blood or blood products, history of a needle stick exposure, and ethnicity/country of birth from areas of the world with a high rate of HBV endemicity. HBsAg Risk Cascade testing reflexes into two different algorithms depending on if reported as Negative (Non-Reactive) or Positive (Reactive).
