Actinomycetes-Aer/Anaer/GS/Fungal

Message
If more than 4 organisms are recovered from a single specimen taken from 1 site, then only Gram stain reaction and morphology of each organism will be reported, unless attending physician specifically requests laboratory to identify them.

A positive bacteria result, depending on the organism isolated, may reflex additional identification and/or susceptibility charges.

Instructions
Submit only 1 of the following specimens:
Blood Adults Transfer 8 mL to 10 mL (minimum volume: 3 mL) of blood to a BACTEC Plus Aerobic/F culture bottle and 5 mL to 7 mL (minimum volume: 3 mL) of blood to a BACTEC Standard Anaerobic/F culture bottle. Infants/Children/Difficult to Draw Patients Transfer 1 mL to 3 mL (minimum volume: 0.5 mL) of blood to a BACTEC Peds P1us/F culture bottle.
Recommended Volumes Adults: 13 mL to 17 mL per venipuncture
Children: 1 mL to 5 mL per venipuncture
Neonates: >1 mL per venipuncture
Draw specimen as follows: 1. Pinch the wings of a ChloraPrep One-Step Frepp Applicator (2% chlorhexidine gluconate/70% isopropyl alcohol) to break its ampule and release the antiseptic. Thoroughly cleanse the puncture site by pressing the sponge against the skin and scrubbing in a back and forth motion for 30 seconds. 2. Allow 30 seconds for the venipuncture site to dry. 3. Do not palpate vein to be entered unless palpating finger is covered with a sterile glove or has been prepared in same way as puncture site. Note: If the patient is <2 months of age, cleanse the puncture site with 70% alcohol, followed by 30 seconds of scrubbing in a circular motion with tincture of iodine. Allow at least 2 minutes for the iodine to dry before venipuncture. Obtain blood and transfer to a BACTEC Peds Plus/F culture bottle and deliver immediately to laboratory.

Bronchial Brush 1. Collect bronchial brush specimen in a screw-capped, sterile container. 2. Maintain sterility and forward promptly. Note: Specimen source is required on request form for processing.
Spinal Fluid 1 mL of spinal fluid collected using a Pharmaseal Lumbar Puncture Tray. Note: Specimen source is required on request form for processing.

Sputum 1. Collect specimen in a screw-capped, sterile container. 2. Specimen should be a single, first-morning, ""deep cough"" sputum specimen. Patient should not have eaten prior to collection. When antibiotics are to be started immediately, a first-morning specimen is still preferred. 3. Instruct patient to brush his/her teeth and/or rinse mouth well with water to remove any indigenous flora from mouth area. 4. Have patient remove dentures. 5. Instruct patient to take a deep breath, hold it momentarily, and cough deeply and vigorously into container. Close lid securely. 6. Maintain sterility and forward promptly at ambient temperature only. Note: 1. Cough induction by inhalation of a saline aerosol may be used if required. 2. Specimen source is required on request form for processing.

Sterile Aspiration 1. Decontaminate collection site with 70% alcohol. 2. Inject a small amount of sterile saline, if required, to reduce consistency enough to aspirate. 3. Puncture skin with needle and aspirate material from lesion. 4. Remove needle from syringe, seal syringe containing aspirate, and deliver to laboratory immediately. Note: Specimen source is required on request form for processing.

Tissue 1. Decontaminate collection site and aseptically collect 0.5 g of tissue and place in a screw-capped, sterile container. 2. Maintain sterility and forward promptly at ambient temperature only. Note: Specimen source is required on request form for processing.
