    • 2 patient identifiers (i.e. entire name & date of birth)
  • Requisition submitted with samples must contain:
    • Biopsy site (or list of sites for multiple specimens)
    • Correct patient information
    • Collection date and time
    • Physicians name
    • Clinical diagnosis
    • If available, brief history and patient information
