Clinical Significance
Tacrolimus (FK506, Prograf) is a calcineurin inhibitor that blocks activation of the transcription-regulating factor, preventing transcription of the gene coding for IL-2 and blunting T-cell activation.  It is a widely used drug for bone marrow and organ transplantation, administered early after transplant and later for maintenance therapy.  The goal for maintenance therapy is to achieve optimal immunosuppression while minimizing dose-dependent toxicity.  Peak levels are reached 1-2 hours after oral dose.  Maintenance therapeutic levels are often collected according to trough values desired by clinical protocol.
