Includes
Case to be accessioned in Pathology. Peripheral Blood Smears prepared by Hematology Department.

Most recent CBC Differential order to be linked for correlation of clinical findings.

May reflex for additional Flow Cytometry or Molecular Studies, where indicated or recommended by Pathologist assigned to the case.


Preferred Specimen
This order is preferred to be added on to an existing CBC with Differential within last 24 hours unless patient’s clinical presentation warrants a new specimen to be collected for peripheral smear review.


Minimum Volume
1.5 mL
For the peripheral blood smear and potential reflex tests if indicated


Other Acceptable Specimens
New sample: 2.5 mL whole blood (lavender top tube, EDTA)


Instructions
Provider to indicate clinical indication for Pathologist review when placing order.

Hematology Department to prepare peripheral blood slides from most recent CBC Differential order. Hematology Department will result test from the outstanding list in LIS with ‘Yes.’

Pathology label will generate after final verifying the ‘Yes’ response. Deliver 2 smears and the AP generated label to pathology.

Refrigerated 48 hours
Allow refrigerated samples to come to room temperature and mix well before slide preparation

Smear referred to Pathology Department for review and reporting.


Limitations
Many abnormal morphological findings in patient peripheral blood smear can be detected by routine CBC with Differential order in the Hematology Department. In the possibility that a true hematological disorder is present, a manual review of the patient’s peripheral blood smear will be performed, and findings reported to patient’s chart by the Hematology CLS or escalated further to a Hematopathologist, where indicated per policy.


Clinical Significance
Suspected Hematological disorders can be reviewed by Pathologist and consulted with provider accordingly. Further studies may be recommended or reflexed after preliminary review is completed by the Pathologist.
