Instructions
To rule out Coagulation Factor deficiencies. Special order by physician ONLY.
*NOTE: If ordering with Protime/INR, must order the Mixing Studies using Priority "STAT" to prevent duplication of components.
LAB: Centrifuge in validated Platelet Poor Plasma Centrifuge for 2 minutes at 7200 rpm. Specimen to be delivered to Coagulation department STAT. Notify CLS upon arrival.
**Testing must be completed within 24 hours of collection.
Note: If unable to deliver whole blood to testing laboratory within stability time, platelet poor plasma should be aliquoted and placed in freezer. Centrifuge light blue-top for 2 minutes at 7200 rpm (STAT Coag centrifuge) within 60 minutes of collection. Using a plastic pipette, remove plasma, taking care to avoid the WBC/platelet buffy layer and place into a plastic vial. Centrifuge aliquoted plasma a second time and transfer platelet-poor plasma into a new plastic vial and freeze at -20℃. Once thawed, specimen must be tested within 2 hours.
Freeze aliquot at -20℃ for at least 1 hour when sending to alternate campus for testing.

Methodology
This is a 1:1 screening procedure (with no incubation mix reflex) to determine whether a prolonged Protime or APTT is caused by a Factor Deficiency or the presence of an inhibitor. If a 50% dilution of the patient's plasma with normal plasma corrects the prolonged test (APTT or PT), it is suggestive that the normal plasma has supplied the missing/deficient factor, but does not rule out the presence of an inhibitor since there is no incubated mix reflex testing performed. If the prolonged test does not correct on 1:1 mix, an inhibitor or multiple factor deficiencies may be present in the patient's plasma.

Rosner Index result is < 12. Use Rosner Index (RI) Formula:

12 - 15: Indeterminate; retest
> 15 Presence of Inhibitor "Correction" indicates single or multiple factor deficiency(ies).
