Instructions
1. Call GBMC's Surgical Pathology Department (443-849-2812) at least 48 hours in advance of the procedure to obtain the recommended operative protocol and to arrange for operating room assistance by pathology personnel. 2. Label specimen with patient's name (first and last), date of birth or history number, name of requesting physician, and date and source of specimen.Collect segment of nerve measuring 1.5-2.0 cm in length.
NOTES: Place order for Surgical Pathology Exam in EPIC or complete a "Tissue Examination Request Form" and abstract of clinical history which is necessary for correlation and forward them with the specimen. These forms are supplied by GBMC.
