Instructions
Label body with patient's name (first and last), date of birth, history number and name of admitting/attending physician.1. Perform appropriate postmortem care as per hospital policy. 2. In possible medicolegal cases, the State Medical Examiner's Office must be contacted before any suggestions are made to the family of the deceased. The Admitting Office is aware of the name of the medical examiner and how he can be reached. The medical examiner may rule that an autopsy need not be performed by his office. This is not a transfer of authority. To perform an autopsy in this situation, a properly signed permit must be obtained. 3. Permission for autopsy may consist of a written document, telegram, or facsimile transmitted authorization by the legal next of kin or other legally authorized person. Telephone permission is not acceptable. 4. An improperly executed "Autopsy Permit" is cause for rejection.
