Clinical Significance
Infectious mononucleosis is an acute or subacute benign infectious lymphoproliferative disease that is, associated with the Epstein-Barr virus of Burkitts lymphoma. The disease is characterized by an increase in atypical lymphocytes in the circulation, by enlargement in the lymph nodes and spleen, by fever and by characteristic heterophile antibodies. Heterophil antibodies for sheep red cells are found in about 80-100% of patients with infectious mononucleosis. They are IgM 19S immunoglobulins, which can be demonstrated between the fourth and tenth day. Absence of sheep cell-agglutination antibodies does not exclude the diagnosis of infectious mononucleosis, but even high sheep cell titers are not diagnostic of infectious mononucleosis since they have been reported in other myelolymphoproliferative disorders such as leukemia, polycythemia vera, Hodgkins disease and others.
