Clinical Significance
The Activated Partial Thromboplastin Time test is used to monitor deficiencies or changes in the intrinsic pathway of coagulation. Extracted cephalins are used as a source of phospholipids to replace the platelet factor3 released during coagulation in vivo. Particulate activators enhance the activation of the contact factors of the intrinsic pathway: Factors XII, XI, high molecular weight kininogen (HMWK), and prekallikrein (PK). The APTT is used to test all factors of the intrinsic and common pathways, including factors, I, II, V, VIII, IX, X and the contact factors. In addition, it is sensitive to certain inhibitors of coagulation such as heparin, lupus anticoagulant, and specific factor inhibitors. The Activated Partial Thomboplastin Time is the most commonly used test to monitor heparin therapy.
