Instructions
Label container with patient's name (first and last), date of birth or history number, name of requesting physician, and date and source of specimen.1. Forward all excisional biopsies, lumpectomies, mastectomies and sentinel lymph nodes to the Surgical Pathology Department immediately after excision. 2. Needle-localized breast biopsies should be x-rayed, then delivered to the Surgical Pathology Department. 3. Record the time of excision in military time on the specimen label and/or pathology requisition.
