Instructions
Accurate specimen labelling is imperative to avoid transfusion errors. Label must include patient's full first and last name, phlebotomist's full name (id ok for CHOMP employees), patient's date of birth and Blood Bank wristband number.

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Excessive hemolysis and/or lipemia, specimen frozen, contaminated or label not complete.
