Instructions
Collect after the infant is at least 12 hours old.
Use the CDPH Newborn Screening Test Request Form (TRF) and follow instructions for collection on the form. Use capillary collection only and completely fill out the circles on the form.
Enter Collection information in Epic - do not write on TRF form except for parental refusal
Allow to air dry for 3 hours.
Keep away from direct sunlight or heat.
Do not allow wet specimens to come into contact with each other.
Do not put in a plastic bag at any time.
