Test CodeLAB63
Alias/See Also
CPKMB
CPT Codes
82550, 82553
Includes
CK-MB always includes CK
Instructions
Collect one 4.5ml light green top (heparin) tube or one 5ml Red/gold top serum separator tube,
Specimen Stability
Refrigerate, stable 8 hours; stable 4 hours at room temperature; stable 2-3 days if frozen.
Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Insufficient quantity; gross hemolysis, lipemia, or icterus; failure to follow stability requirements
Methodology
"CPK: Multiple-point rate:The rate of change in reflection density is then converted to enzyme activity. CKMB: Immunometric:The amount of HRP conjugate bound is directly proportional to the concentration of CK-MB present."

Clinical Significance
After an acute MI (myocardial infarction) the CK-MB leaks into the circulation reflecting damage to the myocardium. CK-MB rises rapidly to peak levels within 12 hours, then declines to normal levels within 36-72 hours. Elevated CK-MB levels are also associated with skeletal muscle trauma but do not have the rise and fall characteristics of acute MI CK-MB levels.
