Clinical Significance
Pentobarbital (Nembutal®, Wigraine-PB®, WANS®, Thiopental metabolite) is a short acting barbiturate, which is used
principally as a preoperative sedative/hypnotic and muscle relaxant and as an inducer of coma for the treatment of
intracranial pressure due to head injuries. It is also used in the treatment of barbiturate withdrawal. The sedative dose
for Pentobarbital is 20-40 mg, 2-4 times a day; the hypnotic dose is 100-200 mg at bedtime. Peak plasma levels occur
within 30-60 minutes after an oral dose and coincide with the onset of action. Duration of action is 1-4 hours. Side
effects include CNS depression, confusion, drowsiness, ataxia, coma, respiratory depression, shock, and urticaria. Abuse
produces tolerance and physical dependence.
