Instructions
Special kit will be given to patient by provider. Patient will drop off completed kits to the lab for sendout to VA Baltimore.
1. Collection kits for Depleted Uranium and Toxic Embedded Fragments are obtained by calling the Baltimore VA (800) 815-7533. This number is given in the handbooks and on the DU Program Specimen Collection Checklist. Kits will include a Questionnaire and the Checklist which must be faxed back to Baltimore and a copy included with the specimen. At some sites the OIF/OEF coordinators have kits and give them out to the patient directly. Other sites direct the patient to the lab for the kits. Please iron this out locally.
2. It is a VISN 1 decision to have the results go to the lab (Aug 2012 NCL EC). Per Jennifer at DU Program ,Baltimore: On the DU Checklist, sections 13 & 14 Referring facility contact name & number - this is usually the OEF/OIF coordinator - add a note in this section to "send copy to lab" and include your fax number so results will be faxed to lab also. You will need to do this for each specimen sent.
