82040, 82042, 82784 (x2), 86618 (x4)

Preferred Specimen
2 mL serum collected in a serum separator tube (SST) and
2 mL CSF collected in a leak-proof CSF container, within a 24-hour period

Minimum Volume
1 mL serum • 1 mL CSF

Instructions
2 mL serum and 2 mL CSF collected within a 24-hour period

Transport Temperature
Room temperature

Specimen Stability
Room temperature: 7 days
Refrigerated: 14 days
Frozen: 30 days

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
CSF and serum collected greater than 24 hours apart

Methodology
Enzyme Linked Immunosorbent Immunoassay (ELISA) • Nephelometry

Setup Schedule
Set up: Tues, Fri; Report available: 2-6 days

Reference Range
Lyme Disease Antibody Index
≤1.0   Negative
1.1-1.2   Equivocal
≥1.3 Positive

Control Antibody Index ≤1.0
Albumin Ratio <0.0078

Clinical Significance
The Lyme disease antibody index is used as an aid for diagnosing neuroborreliosis. The assay requires paired serum and CSF. A result set of lyme disease antibody index >1.2, control antibody index <1.0, and albumin ratio <0.0078 provides strong evidence of intrathecal synthesis of b. Burgdorferi antibodies and supports the diagnosis of neuroborreliosis.
