Methodology

Limitations
Routinely, the assay will be performed only on samples that have a viral load higher than 1,000 copies/ml, because the sequencing of samples with viral load lower than 1,000 copies is not consistently successful. If a recent HIV RNA viral load (1-2 weeks) is not available, a viral load test will be ordered and performed before the sample is tested for HIV genotyping. Genotyping of samples with viral load lower than 1,000 copies will be attempted only if specifically requested.

Clinical Significance
HIV-1 genotyping assays are recommended for patients who experience treatment failure on their first or subsequent regimen. HIV genotyping should also be considered in patients with primary infection, and in patients with established infections that have not yet been treated.
