For COMPLICATED INFECTIONS such as bacteremia, endocarditis, meningitis and hospital acquired pneumonia caused by S. aureus, recommended TROUGH level is 15-20 ug/mL. For uncomplicated infections, troughs of 10-15 ug/mL may be adequate. Trough therapeutic range was increased to address evidence based guidelines for Vancomycin use aimed at improving efficacy of antibiotics for serious infections & reducing emergence of resistant strains.

Clinical Significance
Vancomycin is an antibiotic used in the treatment of serious bacterial infections, especially methicillin-resistant staphylococci. Vancomycin is also used in treatment for penicillin-allergic patients who cannot receive or have failed to respond to other drugs, including the penicillins and/or cephalosporins and for other infections caused by vancomycin-susceptible organisms.
