Age-dependent points for acute heart failure:
<50 years: >450 pg/mL
50-75 years: >900 pg/mL
>75 years: >1800 pg/mL

Values above the age-defined reference range have high positive predictive value for the diagnosis of acute heart failure in the absence of renal failure. Values below 300 pg/mL have a high negative predictive value to exclude acute heart failure. Values between the ranges are indeterminate.

Non-acute setting

HF unlikely
Consider other diagnoses.
 
<75 years with value >/= 125 pg/mL   or            
>75 years with value >/=450 pg/mL                           
HF likely
Consider other confounding factors and perform echocardiography to confirm HF diagnosis.
