Place order in Meditech.  Provide required essential information that includes pertinent clinical history (blank, "none" or "N/A" are not acceptable).  A manual Pathology Specimen Request form can be used when you do not have access to a computer and/or during downtime.  Specimens collected after hours must be refrigerated until processing.  Indicate in the operative procedure to R/O Pneumocystis carinii if applicable
