Preferred Specimen
Submit fresh tissue.  Do not add formalin.


Instructions
Submit fresh specimen to the laboratory along with a completed Pathology Specimen Diagnosis Request form.  Pertinent clinical history is required.  Do not add formalin.  For frozen sections, the procedure must be scheduled in advance with the pathology department to ensure a pathologist is available.  When tissue for frozen section is submitted via the PEVCO tube, the blue frozen section form must be visible in the biohazard bag and the lab is to be notified to alert them that a STAT specimen has been sent.  For all special studies,  clearly indicate the test requested on the manual requisition.


Transport Container
Clean specimen container.  When microbiology is requested, a sterile container is required.
