Container/Tube: Container with 10% neutral buffered formalin

Collection Instructions:

1. Submit specimen in container with enough formalin to cover specimen. Use of gauze pads is not acceptable.

2. Orient tissue if appropriate.

3. Label container with at least 2 sources of patient identification (eg, bar code, patient name, date of birth, or other unique identifier), date of collection, and type and site of specimen.

Frozen Section

Outpatient frozen sections should be scheduled in advance with a 30 to 45 minute notice, if possible. Cases should be scheduled as early as possible, no later than 3:30 p.m., so they do not extend beyond courier service hours. At the time of scheduling, provide a direct phone number to the attending physician so the pathologist can quickly relay diagnosis. To prevent delays, do not provide an office or answering service number that does not directly call the attending physician.

Inpatient frozen sections before 8 a.m. or after 5 p.m., call SaraPath Diagnostics’ answering service at 941-362-8900 to contact a pathologist.

Specimen Type: Frozen section

Container/Tube: Container with no preservative

Collection Instructions:

1. Submit fresh specimen. No preservative. Use of gauze pads or specimen in formalin is not acceptable.

2. Orient tissue if appropriate.

3. Label container with at least 2 sources of patient identification (eg, bar code, patient name, date of birth, or other unique identifier), date of collection, and type and site of specimen.

Note:

1. Complete a SaraPath Diagnostics requisition, include all relevant clinical information, and submit with specimen.

2. Verify patient name and other unique identifiers on requisition match those on specimen.

3. Specimen may be rejected and returned to originating site if any of the following conditions apply:

a. Patient or specimen identification errors or omissions

b. Incorrect specimen submitted for test requested

c. Damaged or leaking specimen

Note: STAT Specimen - Call for pick-up of STAT specimen and indicate STAT on requisition.
