Preferred Specimen
Submit only 1 of the following specimens:

Catheterized Specimen
Container/Tube: Sterile container(s)-Foley catheter tip is not acceptable.
Specimen Volume: 2 mL of urine
Collection Instructions: 2 mL of fresh urine from an indwelling catheter tube through sampling port as follows: 1. Avoid sending urine that has remained stagnant in catheter tubing for any length of time or from catheters that have been in place longer than 5 to 9 days. 2. Do not collect urine from drainage bag. 3. Clean sampling port with an alcohol swab. 4. Pour urine into a sterile container. Mix well. 5. Label container with patient’s name (first and last) and date and actual time of collection.
Note: 1. Specimen source is required. 2. Two separate specimens are required if urinalysis is also requested.  3. Routine processing of urine from patients with chronic indwelling catheters may be of no value except epidemiologically. Large numbers of potential pathogens are common in these patients.

Midstream Specimen
Collect specimen prior to antibiotic administration because most antibiotics concentrate in urine.
Container/Tube: Sterile container(s)
Specimen Volume: 2 mL of urine
Collection Instructions: 1. 2 mL of urine from a first-morning specimen collected as follows:
Males:
     A. Wash and dry hands with soap.
     B. Foreskin must be completely retracted and glans penis is cleansed with soapy water. Surplus soap is removed with warm, sterile water.
     C. Patient should void approximately 20 mL to 25 mL of urine into toilet and then pass a portion of midstream urine into a sterile container.
Females:
      A. Wash and dry hands with soap.
      B. With 1 hand, spread labia and keep them continuously apart until urine is voided.
     C. Taking a saturated soapy sponge, wash vulva well, passing from front to back. Discard sponge. While vulva is still spread, repeat front to back wash with remaining sponges, discarding each as its use is finished. Patient should wash with warm sterile water to eliminate possibility of contaminating specimen with soap.
     D. Patient should void approximately 20 mL to 25 mL of urine into toilet.
     E. Collect midstream portion of urine into a sterile container. Cup should be held in such a way that contact with legs, vulva, or clothing is avoided.
2. Label container with patient’s name (first and last) and date and actual time of collection.

Limitations
Note: Bacteria present in <1,000 organisms/mL are not detected by this method. This test will not detect uncommon agents such as anaerobic bacteria, fastidious organisms, mycobacteria, mycoplasmas, chlamydia, etc.

Clinical Significance
Useful for isolating and identifying aerobic pathogenic bacteria causing urinary tract infection.
