Preferred Specimen
Submit only 1 of the following specimens:
Plasma
Container/Tube: Green-top (heparin) tube(s)
Specimen Volume: 3 mL (minimum volume: 1 mL) of plasma for each specimen
Collection Instructions: 1. Draw a baseline cortisol at least 30-minutes after insertion of stopcock. 2. Draw cortisol 30- and 60-minutes after injection of Cortrosyn. 3. Separate plasma from red cells within 1 hour.
Note: 1. Do not begin test immediately after Cortrosyn administration. 2. Draw all specimens from stopcock. 3. Indicate plasma and as 0-, 30-, or 60-minutes. 4. Label specimen appropriately (plasma and as 0-, 30-, or 60-minutes).
Serum
Container/Tube: Serum gel tube(s)
Specimen Volume: 3 mL (minimum volume: 1 mL) of serum for each specimen

Normal response is a rise in serum cortisol >=7 µg/dL and peak response >20 µg/dL
Clinical Significance
Useful as an adrenal function test, for diagnosing Addison’s disease, and primary adrenal hypofunction. It is also used as a means of differentiating between hypopituitarism versus hypoadrenalism
