Instructions

Collect random specimen in a sterile urine cup or collect timed specimen in a 24-hour urine jug.  A 2-hour timed collection is the preferred specimen.  For timed specimens:  Record start and stop time or number of hours collected on jug.  If transferring specimen from larger jug to a smaller cup, measure volume first and record on smaller cup, along with the start and stop time or number of hours.

This test is for Uric Acid measurement on Urine.  For Uric Acid measurement on Plasma or Serum, order URAC.  For Uric Acid measurement on Body Fluids, order MISURAC.

Limitations
Hydralazine, an antihyperintensive agent, causes approximately -2% bias per 1.0 mg/dL of hydralazine.

Urine Volume:                                   800 - 1800 mL/24 hours
Urine Uric Acid/Total Volume Ratio:  250 - 750 mg/24 hours



Clinical Significance

Urine Uric Acid measurement is useful for assessment and management of patients with kidney stones, particularly uric acid stones.

Uric acid is the end-product of purine metabolism. It is freely filtered by the glomeruli and most is reabsorbed by the tubules. There is also active tubular secretion.

Increased levels of uric acid in the urine usually accompany increased plasma uric acid levels unless there is a decreased excretion of uric acid by the kidneys. Urine uric acid levels reflect the amount of dietary purines and also endogenous nucleic acid breakdown.

Urinary uric acid excretion is elevated in a significant proportion of patients with uric acid stones.

Uric acid excretion can be either decreased or increased in response to a variety of pharmacologic agents.

Urine uric acid levels are elevated in states of uric acid overproduction such as in leukemia and polycythemia and after intake of food rich in nucleoproteins.
