Clinical Significance
Chloride is the major extracellular anion. The majority of ingested chloride is absorbed and the excess excreted into the urine.
Low levels of Cl- are observed in excessive vomiting, over-hydration, inappropriate ADH secretion, compensated respiratory acidosis, Addison's disease, salt-losing nephritis, severe burns, and in diabetic ketoacidosis.
Elevated Cl- is seen in mineralocorticosteroid deficiency, non-gap metabolic acidosis, excessive amounts of IV infusion with normal saline, renal tubule loss, and in some cases of hyperparathyroidism.
Chloride measurement is useful in differentiating acidemias and alkalemias.
